PKV Premium Relief Tariffs: How Beitragsentlastung Works

Private health-insurance contributions do not simply disappear when you retire. A Beitragsentlastungstarif (BET), also called a PKV premium-relief tariff, can be one contractual tool for reducing the health-insurance premium from an agreed age.

It is not automatically the right choice for every privately insured person. The relief amount, start age, ongoing contribution, employer-subsidy treatment and consequences of changing your plans depend on the tariff and your circumstances. The key distinction is simple: a contractually defined relief component is not a guarantee that your total future PKV costs will be low, fixed or affordable.

What is a PKV Beitragsentlastungstarif?

A Beitragsentlastungstarif is an optional additional component alongside a private health-insurance contract. You pay an additional contribution during the years before the relief age. In return, the tariff is designed to provide a defined reduction in the qualifying health-insurance premium from a later age.

The structure is not identical across insurers. The PKV Association's overview of Beitragsentlastungstarife explains that relief commonly begins around age 65, but the start age can be earlier or later. It also notes that the contribution and the later relief are governed by the particular tariff design.

For that reason, do not treat a BET as a personal savings account. It is a health-insurance tariff component with a specific contractual purpose. The relevant documents are the offer, tariff description, general insurance conditions and any schedule showing the expected relief amount.

What does the tariff actually guarantee?

The safest way to describe the benefit is as a contractually defined premium reduction under the applicable tariff conditions. That is different from fixing the total PKV premium for life.

Private health-insurance premiums are not calculated directly from a person's income, but they can change under the legal rules for premium adjustments. Section 203 VVG allows premium changes when relevant calculation factors change and the statutory requirements are met. A BET may therefore reduce a qualifying part of the premium while the remaining health-insurance contribution can still be adjusted.

The relief component itself also needs to be read carefully. The PKV Association explains that the contribution may be increased or reduced in some tariff structures, with corresponding effects on the later relief. The age at which relief begins, the maximum relief amount and the way adjustments work are not universal PKV rules.

A useful question is therefore not “What will my total PKV premium be?” but “What exact amount is defined, from what age, for which part of my cover, and what can still change?”

Can your employer contribute?

For employees, a qualifying BET contribution may be relevant to the statutory employer subsidy for private health insurance. This does not mean that an employer automatically pays half of the BET.

Section 257 SGB V sets the calculation principle. The subsidy is linked to the contribution that would result from the statutory reference calculation and is limited by the employee's actual eligible private health-insurance cost. The normal PKV premium may already use some or all of the available subsidy capacity.

The practical result depends on at least three points:

  • whether the insurer and the relevant cover satisfy the statutory requirements;
  • how much of the applicable employer-subsidy ceiling is already used by the main PKV tariff; and
  • how the BET contribution is treated in the specific calculation.

The PKV Association describes BET contributions as employer-subsidy eligible, but the statutory cap and the employee's actual eligible contribution still apply. Contributions for private long-term-care insurance are a separate issue and should not be combined with the health-insurance subsidy without checking the applicable rules.

How is a Beitragsentlastungstarif treated for tax purposes?

Tax treatment should be checked separately from the insurance decision. Section 10 EStG provides the framework for deductible private health-insurance contributions, including the part attributable to cover comparable to statutory basic benefits.

That does not mean every BET component is automatically fully deductible. The treatment can depend on the tariff structure, the qualifying coverage, employer contributions, reimbursements and the individual's tax situation. There is no universal deduction percentage that can safely be promised to every privately insured person.

The BMF Einkommensteuer-Hinweise for §10 are a useful reference for the basic distinction between qualifying health-insurance coverage and other components. They do not make every BET component deductible. Ask a qualified tax adviser to review the actual tariff and your circumstances rather than assuming that every health-insurance component receives the same tax treatment.

What happens when you retire?

When the relief age stated in the tariff is reached, the agreed relief mechanism takes effect according to the contract. The contribution for the relief component may continue during the relief period. The PKV Association explicitly notes this point, but the exact operation should be confirmed in the tariff documents.

The rest of the PKV contribution can still change. A BET does not remove the cost of the main health-insurance tariff, private long-term-care insurance or any other included component. It also does not replace a general retirement-income plan.

A separate issue is the statutory pension subsidy. In 2026, privately insured recipients of a statutory pension may apply for a subsidy toward qualifying private health-insurance costs if the legal requirements are met. According to the Deutsche Rentenversicherung's 2026 information, the figure for privately insured pensioners is currently 8.55% of the gross statutory pension, subject to a maximum of half the actual qualifying health-insurance premium. The subsidy is not an automatic payment simply because someone is privately insured, and the pensioner must apply where required.

The subsidy concerns health insurance. The DRV states that pensioners must pay their private long-term-care insurance contribution themselves. Do not treat the 2026 figure as a promise about later years or as a payment of the entire PKV contribution. The general legal basis is §106 SGB VI.

What if your plans change?

For expats, the contract consequences of changing plans may be just as important as the projected relief. Leaving Germany, returning to statutory insurance, changing private insurers or changing tariffs can affect the BET in different ways.

There is no safe universal rule that the money is always lost, always refundable, always transferable or never transferable. Check the tariff conditions for:

  • cancelling or reducing the relief component;
  • increasing the contribution or the later relief amount;
  • moving to another tariff with the same insurer;
  • changing to another private insurer;
  • leaving Germany or establishing coverage elsewhere; and
  • death before the relief age, refunds, transfer values and any beneficiary rules.

Section 204 VVG provides an internal tariff-change principle for comparable coverage with recognition of acquired rights and ageing provisions, subject to the statutory conditions. It does not guarantee that every BET feature, relief amount or exit right will be preserved in every move. The actual AVB and tariff documents remain decisive.

Beitragsentlastungstarif versus investing separately

A separate investment and a BET serve different purposes. Neither should be declared the universal winner.

  • BET: a contractual premium-relief purpose, possible relevance for an employee subsidy, tax treatment subject to statutory rules, and dependence on the insurer and tariff. The money may be less liquid and less flexible than a separate investment.
  • Separate investing: possible liquidity and investment control, but exposure to market risk and no contractual PKV premium reduction. A separate investment does not automatically receive an employer PKV subsidy.

The comparison should include the expected relief, total contributions before and after relief, liquidity needs, employer-subsidy capacity, tax treatment, market risk and the likelihood that you will keep the relevant PKV contract. When might a Beitragsentlastungstarif be worth considering?

A BET may be worth examining if long-term PKV membership is plausible, the tariff terms are understood, the expected relief would be useful and you can accept the reduced flexibility. It may also be relevant if an employee has confirmed how much employer-subsidy capacity is available.

More caution is appropriate when your length of stay in Germany is uncertain, a future move to another insurance system is possible, liquidity is important or the tariff's exit consequences are unclear. Retirement insurance status and contribution rules can also differ depending on a person's statutory-insurance history; this article is not a KVdR or GKV retirement guide.

The decision should follow a review of the actual contract rather than a universal rule.

Questions to check before signing

  1. At what age does the relief begin?
  2. What exact euro amount is contractually defined, and which premium component does it cover?
  3. Does the BET contribution continue after relief starts?
  4. Can the contribution or relief amount be adjusted?
  5. What happens if you change tariff within the same insurer?
  6. What happens if you change private insurer?
  7. What happens if you leave Germany or arrange insurance elsewhere?
  8. What happens if you later return to GKV?
  9. How much employer-subsidy capacity is actually available?
  10. How is this specific tariff treated for tax purposes?

How this fits into your broader PKV decision

A Beitragsentlastungstarif is a financing and retirement feature within a PKV strategy. It should not decide whether you choose private health insurance in the first place. Eligibility, family planning, long-term affordability, healthcare needs and the consequences of changing systems belong to the broader PKV decision.
