What is actually changing in German health insurance in 2027? The short answer is more specific than many summaries suggest: an administrative notification change is relevant, the statutory calculation of the JAEG receives a special adjustment, and several final figures are not yet fixed.
The important correction: Krankenkassen still have to inform members
The 2027 discussion has sometimes been described as if individual notification of a relevant Zusatzbeitrag change were disappearing. That is not the correct conclusion.
Under §175(4) SGB V, the Krankenkasse's individual information duty continues to apply when affected members must be informed about the relevant Zusatzbeitrag situation. The 2026 amendment changes the permitted format: the information may be sent in a separate letter or through a qualifying electronic document under §37(2a) SGB X.
This format change took effect on 30 July 2026 under the general commencement rule. It is therefore relevant to future notifications, including contribution changes that affect members in 2027. It does not remove the individual information duty or transfer responsibility for discovering changes entirely to the member.
What happens if your Zusatzbeitrag increases?
A higher Zusatzbeitrag can be a reason to review your Krankenkasse, but it does not automatically mean that switching is the right decision. The special termination route connected with a relevant contribution change remains available under the applicable rules.
A useful review can consider the total contribution rate, additional benefits, bonus programmes, service, digital access, language support and your personal requirements. The lowest rate is not automatically the best fit for every employee, family or international professional.

The special JAEG adjustment for 2027
The general annual income threshold for compulsory insurance, known as the Jahresarbeitsentgeltgrenze (JAEG), is calculated through a statutory mechanism. For 2027, the legislation provides an additional adjustment of €3,600 per year, equivalent to €300 per month, within that calculation.
This is an adjustment input, not a final 2027 JAEG. The final annual and monthly threshold should not be stated until the authoritative value has been fixed and published.
The legislation also contains separate treatment for certain employees who were already privately insured and exempt from compulsory GKV membership at the relevant cut-off. The €3,600 adjustment should therefore not be presented as an identical universal rule for every insured person or every JAEG category.
What changes for the 2027 contribution ceiling?
The health-insurance contribution assessment ceiling (Beitragsbemessungsgrenze, or BBG) follows its own statutory mechanism. §223(4) SGB V contains a special 2027 rule linking the health-insurance ceiling to the relevant JAEG calculation and statutory adjustment.
That does not mean that every social-insurance threshold receives the same €3,600 increase. It also does not provide a final 2027 health or long-term-care BBG by itself. The applicable values should be checked once the official figures are available.
What is not yet final for 2027?
At the time of this update, the final official values used for the following items have not yet been locked in the authoritative sources used for this article:
- the 2027 JAEG,
- the 2027 health-insurance and long-term-care BBG, and
- the 2027 average Zusatzbeitrag.
The average Zusatzbeitrag for the following year is determined under §242a SGB V and published by the Federal Ministry of Health by 1 November. Until those values are official, forecasts and exact threshold claims can create a misleading impression of certainty. What should employees and expats do?
You do not need to change your health insurance simply because the 2027 rules are being discussed. A sensible checklist is:
- Keep your contact details with your Krankenkasse up to date and read contribution information you receive, whether by post or through an approved electronic document.
- Know your current Zusatzbeitrag and check your payslip if the amount deducted changes.
- If your contribution changes, compare the complete package rather than looking only at the rate.
- Check the applicable special termination and switching deadlines before taking action.
